This form provides our team with the crucial demographic details necessary to reconcile two records in Advance. Additionally, it provides us with pertinent details to better equip our team to research, and eventually merge and purge records at the request of our colleagues. Complete this form to request a merge and purge of two or more records. Advancement Services will complete the research and reconciliation of the records within two business days of request.
